To start setting up a new food safety instruction and testing program, download the Food Safety Proctor Manual
Then complete the following forms:

Mail, overnight or fax the Proctor Application Form, Affidavit of Non-Disclosure Form, and
the completed Proctor Competency Exam to:

After review and approval, a proctor number will be issued. This proctor number must accompany
